Parent/Guardian (child under 18):

Complete and submit our online enrollment form prior to your child's visit.

To complete your enrollment, please print and sign the following liability waiver, survey and the medical release form and bring them with you on your first visit to GoKids.
*If you checked "yes" to any of the health conditions on the enrollment form, you will need to have a physicians clearance form signed by your doctor, prior to your child's participation.
