**When signing up for the GoWeek program please make sure you fill out both a registration form as well as the enrollment form.
Ready to join? It's easy. Here's how:
Parent/Guardian (child under 18):

Complete and submit our online enrollment form prior to your child's visit.

To complete your enrollment, please print and sign the following liability waiver and photo release form and the medical release form and bring them with you on your first visit to GoKids.
*If you checked "yes" to any of the health conditions on the enrollment form, you will need to have a physicians clearance form signed by your doctor, prior to your child's participation.
